  • A complimentary assessment of your care needs is taken so we can better understand your situation, preferences, and needs.

  • We provide information on the different levels of care options.

PRESENTATION OF BEST CARE OPTIONS

  • Living options are presented based on the answers to your complimentary assessment.

  • We review current care situations to make sure all of your needs will continue to be met. 

  • We schedule tours for families to learn more about the living options presented.

PLACEMENT GUIDANCE AND COORDINATION

  • Guidance is provided throughout the transition to ensure your needs are met throughout the moving process.

  • Communication is provided to caregivers, senior living communities and medical providers to ensure a smooth transition of services. 

  • Transition follow up is provided to ensure your care needs are being met.
